visit, or participation, in events held by West Oneonta
Baptist Church we may be photographing you or your
children. We have created this web site and online
photo album to share these photos and would like your
permission to include you and your family. If you would
like your family to be included - please print,
complete, and return this document to WOBC.
Note: First
names MAY be listed in the description of a photo.
Last names or other identifiable information will NEVER
be posted. If you would prefer that your child's
first name not be listed as well, there is a box to
check on the form.
Permissions for use of Olan Mills Portraits:
The Portrait Viewer
CD-Rom is provided with a license from Olan Mills, Inc.
to use the images only in customer's internal data and
record keeping operations. For as long as Olan
Mills is the provider of the customer's pictorial
directory the customer may also display and use the
images on their website and in other church related
work.
